How to get your qma?

• Be at least 18 years of age.
• Have proof of a high school diploma or equivalent.
• Be in good standing as a Certified Nurse Aide (CNA) on the Indiana Aide Registry.
• Worked a minimum of 1,000 hours as a CNA within the last 24 months.
• Complete the 100 hour QMA Training Program.

To be certified as a QMA an individual has to complete additional training and demonstrated competency while dispensing and passing medications and or applying/administrating treatments. In addition, a QMA must obtain six (6) hours of medication related inservice education each year.

Class 1 will be at our classroom, Classes 2-7 online on your time schedule  (You will be receiving your class schedule (syllabus) 1ST CLASS & QMA Book and more)

1.Print out of IN Active CNA certification COPY

2.COPY of HS Diploma, GED, Transcripts

3.Letter from employer(s) HOURS worked as CNA Total HOURS must be 1000 in last 2 years LTC or assisted living. We have a form

4.Letter from employer that you may do clinicals at your job (You must find own clinical site) we have a form

5.TWO TB tests (can be from job, or get two)

6.State of IN Limited Criminal Background Check (can get at IN state police website or from job)

7. COPY of your picture ID (driver’s license)

8.PAYMENT IN FULL OR WORK ONE VOUCHER

IF STUDENT DOES NOT HAVE ALL THE ABOVE, MUST WAIT UNTIL NEXT START DATE TO BEGIN QMA CLASSES

Remainder of schedule and class dates will be given to students at class 1. Students will also complete enrollment paperwork and take a basic math / comprehension test at class 1.

-Active CNA certification in the state of Indiana

-Minimum age: 18 years old

-An official copy of GED/High School Diploma (must be stamped, signed, or marked official)

-Currently employed at a healthcare facility

-Read and write the English language

-Proficient in basic math calculations

-Copy of most recent TB within 1 year

-Work Release form or copy of Physical within 1 year

-Must be able to pass a background check with no felonies within the last five years

-Employment at your clinical site and approval to complete Practicum Hours - This is MANDATORY and will prevent successful completion of the program

-ALL required paperwork will need to be turned in by the first day of class
