I’ll give a slightly different perspective because I made mistakes when I filled mine. The form is divided into four major parts: 1. Personal details – straightforward, just fill from your Aadhaar/PAN. 2. Mode of operation – this is where I got confused. If you’re not sure, ask the staff. I ticked “Jointly” by mistake, which meant both of us had to sign every cheque. Later I had to submit modification request to change it to “Either or Survivor.” 3. Nominee details – don’t skip this! Nominee ensures smoother settlement in case of unforeseen circumstances. 4. KYC declaration – tick all relevant boxes. My advice: fill with a black pen, keep it neat, and double-check everything before submission.
