You can apply for PM-JAY (Ayushman Bharat) by checking if you are eligible on the official website. Just visit https://mera.pmjay.gov.in, enter your mobile number and OTP, and verify with your ration card or Aadhaar. If you are eligible, you can get the e-card from the nearest CSC (Common Service Center) or Ayushman Mitra in government hospitals.

I applied offline through my village panchayat office. They had a PM-JAY desk where I gave my Aadhaar and ration card. Within a week, I got my Ayushman card printed. Best part—it covers up to 5 lakh per year for my whole family.
