In my personal experience, when I had too many white spots, my dermatologist suggested improving diet with more zinc, protein, and hydration. After 2–3 months, my nails were clearer. So diet does play a role sometimes.

I’d add psychological barriers. If you’re stressed, anxious, or angry, you cannot focus properly on the speaker. Similarly, if you already assume you know what they’re going to say, you stop listening attentively.
