For residents who have splints and need Passive Range of Motion (PROM) the easiest approach is to incorporate the program with the application and removal of the splints. This should be accomplished by the nursing staff as part of the routine daily care. For residents who need more Range of Motion (ROM) and have not been identified as part of the Exercise Programs, the AROM or PROM programs can also be incorporated into the daily routine just like the Activities of Daily Living (ADL) programs. The smoothest way to launch this is to incorporate the programs into the resident’s AM and PM care. Grab your therapy and clinical leaders and do some education with the staff on what these programs look like. Make it fun and be sure to emphasize that this isn’t another thing but really an inclusive part of routine care. ROM can be done in as simple as providing the ROM while getting dressed.

ADL Set-Up and Prompt: Hygiene & Grooming

Appropriate Restorative Program: ADLs

Purpose and Description: Improve independence in activities of daily living. Enable patient to do for self with set up and prompting from facility staff. Patient ADL abilities may vary based on cognition and comorbidities.

Overview:

The intent of this restorative program is to assist the resident in the independence of their personal hygiene. This could include planning the task and gathering the supplies, combing or styling their own hair, washing their face and hands, brushing teeth, shaving or make-up application, or application of deodorant.

The intent is for the staff not to dress the patient, but for the patient to be actively involved in dressing themselves.

Overview:

Always encourage the patient to do as much as possible for themselves. This may take more time, but it is more important in helping the patients care for themselves.

Passive Range of Motion

Purpose and Description: To help keep a patient’s joints flexible mitigate the risk of contractures for quality of life. During passive range of motion, staff is creating motion for the resident that engages their muscles they would normally use to start movement. Doing these exercises for patients helps with muscle performance, range of motion, mobility, and overall health.

Lower Extremity Passive ROM Exercises are for someone to stretch the patient’s hips, legs, and/or knees when patient is unable to do his or herself. These exercises should be done slowly and gently while patient is lying on his or her back, in conjunction with morning or evening ADL care, or with the application and removal of splints or braces.

Upper Extremity Passive ROM Exercises are for someone to stretch the patient’s arms when patient is unable to do his or herself. These exercises should be done slowly and gently while patient is lying on his or her back, sitting in a chair, in conjunction with morning or evening ADL care, or with the application and removal of splints or braces.

Guidelines for Range of Motion (AROM or PROM)

General Principles

o   Begin range of motion as soon as possible after direction received from the rehabilitation nurse or therapist

o   Use principles of good body mechanics

o   Complete movements in a smooth, slow, gentle, and rhythmic fashion

o   Complete each movement five times during an exercise period

o   Incorporate into the bathing routine

o   Complete while resident is in sitting or side lying position

o   Teach the resident to do their own ROM as soon as possible

Method for Passive ROM

o   Place one hand above the joint to provide support

o   Use the other hand to gently move the joint

Return Demonstrations for Training

¨    Form groups of three persons each

¨    One individual will act as the resident

¨    The second individual will complete range of motion following the appropriate     sequence

Residents who are coded as independent in an ADL activity are usually NOT maintenance service can be included when counting restorative nursing minutes. Training and Skill Practice Programs (dressing and grooming) will minimize.
